Finding the cause, and making sure it does not return.
Root cause failure analysis, performance analysis and corrective and preventive action plans for gas turbines, steam turbines, compressors and their auxiliaries.
What it is
Turbomachinery reliability work explains why a machine failed or underperformed, and defines the actions that stop the problem from returning.
A good root cause failure analysis rests on evidence: operating data, inspection findings, and the records of how the machine was installed, commissioned and maintained. When those records are missing or weak, establishing the failure path becomes much harder.
Who it is for
- Owners and operators dealing with a failure, repeat trips or unexplained performance loss.
- EPC contractors and project teams facing a failure during commissioning or warranty.
- Insurers and asset managers who need an independent technical view.
What problems it solves
- Repeat failures where the visible damage has been treated, but not the cause.
- Disputes between parties about what went wrong and why.
- Performance loss that has no clear explanation.
- Corrective actions that are agreed but never verified.
What we offer
Root cause failure analysis
A structured investigation from evidence collection to root cause, with a clear, documented failure path.
Performance analysis
Review of operating and test data against expected performance, to locate where and why the machine falls short.
Corrective and preventive action plans
Practical actions, owners and verification steps, so the fix is confirmed rather than assumed.
How it works
- Introductory call. We discuss the event, the machine and the data available.
- Evidence collection. Operating data, inspection findings, installation and commissioning records, and interviews.
- Analysis. Possible causes are tested against the evidence until the failure path is clear.
- Actions. A report with root cause, contributing factors and a corrective and preventive action plan.
The same solution applies across the four industries we serve: data centers, power generation, LNG, and gas compression and process.
Common questions
What is root cause failure analysis (RCFA)?
RCFA is a structured method to find the underlying cause of a failure, not only the visible damage. It tests possible causes against the evidence until one failure path is supported, then defines actions to prevent recurrence.
What information do you need to start an RCFA?
Operating and trip data, inspection findings and photographs, maintenance history, and the installation and commissioning records, such as alignment reports, check sheets and ITPs. We start with what is available and identify what else is needed.
How long does an RCFA take?
It depends on the event and on access to evidence. A focused investigation can take a few weeks; complex failures that need laboratory work or OEM input take longer. We agree a plan and timeline at the start.
